I understand my reservation fee is non-refundable, but may be transferred to a future litter if requested in advance.

I understand that the final payment is due in cash on Pick-Up Day or via

Zelle at least 7 days prior.

I acknowledge that failure to pick up my puppy on the scheduled date will result in a $45/day boarding fee.

I understand coat color, markings, and size are not guaranteed.

I agree that all changes or communication must be submitted in writing.

I understand that submitting this form does not guarantee a specific puppy and that a non-refundable $500 reservation fee is required to officially reserve my place in line.
